Step 1 Download the Official Form
Use the version provided by your CMRA when available. If not provided, download the official version from the USPS website. Search for Form 1583 and confirm the revision date before completing it.
Step 2 Sections 1 and 2 Mailbox Provider Information
If these sections are pre filled by your provider, do not modify them. If blank, enter the exact legal name and address of your mailbox provider. The information must match your mailbox agreement.
Step 3 Sections 3 and 4 Personal Information
- Select Residential or Personal Use if applicable
- Enter your full legal name
- Enter your phone and email
- Your home address must match your address verification document exactly
Step 4 Section 8 Government Photo Identification
Provide one valid government issued photo ID such as a passport or driver license. Include the ID number, issuing authority, and expiration date. The ID must not be expired.
Step 5 Section 9 Address Verification
Provide a second document that verifies your residential address. Examples include vehicle registration, insurance document, lease agreement, or voter card. This document cannot be the same one used in Section 8.
Step 6 Section 13 Signature and Date
- Sign your full legal name
- Use MM/DD/YYYY format
- Do not leave the signature field blank
