How to Fill Out USPS Form 1583 Step by Step

Step 1 Download the Official Form

Use the version provided by your CMRA when available. If not provided, download the official version from the USPS website. Search for Form 1583 and confirm the revision date before completing it.

Step 2 Sections 1 and 2 Mailbox Provider Information

If these sections are pre filled by your provider, do not modify them. If blank, enter the exact legal name and address of your mailbox provider. The information must match your mailbox agreement.

Step 3 Sections 3 and 4 Personal Information

Step 4 Section 8 Government Photo Identification

Provide one valid government issued photo ID such as a passport or driver license. Include the ID number, issuing authority, and expiration date. The ID must not be expired.

Step 5 Section 9 Address Verification

Provide a second document that verifies your residential address. Examples include vehicle registration, insurance document, lease agreement, or voter card. This document cannot be the same one used in Section 8.

Step 6 Section 13 Signature and Date

Important Do not sign before your live notary session if your provider requires live signing.

After Completing the Form

  1. Save the completed form as a PDF
  2. Take clear photos of your Photo ID and Address ID
  3. Upload securely for online notarization
  4. Receive your notarized Form 1583 by email
