USPS Form 1583 for LLC: How to Complete and Notarize It
If your LLC uses a commercial mail receiving agency (CMRA) or virtual mailbox, you will usually need USPS Form 1583, June 2024 edition. The form authorizes the mailbox provider to receive mail for the business. An officer or other qualified applicant completes and signs the application for the LLC.
Who Completes Form 1583 for an LLC?
An owner, member, manager, officer, or other authorized representative completes the form. Use the individual's legal name where the form asks for the applicant's name and use the LLC's exact registered or operating name where the form asks for the business or firm name.
For Business/Organization Use, an officer must sign and provide a title. Section 12 is used to list additional members who will receive mail at the PMB. Each listed person may be asked to present two valid forms of identification to USPS.
Before You Start
- Your LLC's exact legal or trade name
- The LLC's complete business address and contact details
- The CMRA or virtual mailbox address
- One acceptable government-issued photo ID
- One acceptable address ID showing the signer's current residence
- The applicant's title as an LLC officer or representative
- The names of any additional members who will receive mail
How to Complete USPS Form 1583 for an LLC
| Section | What to enter | LLC tip |
|---|---|---|
| 1. Private Mailbox Information | Enter the date the PMB opened. The date closed is completed later, if the mailbox closes. | Use the date supplied or confirmed by your mailbox provider. |
| 2. CMRA Place of Business Information | Enter the CMRA street address, PMB number, city, state, and ZIP + 4 used for mail delivery. | This is the virtual mailbox location, not the applicant's home or LLC operating address. |
| 3. Type of Service Requested | Check Business/Organization Use for an LLC. | Checking this option means Section 7 must also be completed. |
| 4. Name of Applicant | Enter the individual applicant's name, telephone number, email, complete home address, and protected-individual answer. | The home address in 4f-4j must match the address documented in Section 9. For 4k, answer accurately; if Yes, attach the required court order. |
| 5. Authorized Individual | Complete this only if another person is authorized to pick up mail for the PMB holder. | Enter that person's name, contact details, and complete home address. This is not automatically the same as the applicant. |
| 6. If Transferring PMB Mail | Complete the destination address, telephone number, and email if mail delivered to the PMB will be transferred, mailed, shipped, or emailed elsewhere. | Leave it blank only when it does not apply and your CMRA agrees. |
| 7. Business/Organization Information | Enter the LLC name, type of business, complete business address, telephone number, and place of registration. | For a domestic LLC, the place of registration is the state of formation. For a foreign entity, follow the form's country and state/city instruction. |
| 8. Applicant Photo ID | Enter the applicant's name, ID number, issuing entity, expiration date, and check the photo ID type. | Use an acceptable government-issued photo ID. Attach a copy. A driver's license cannot serve as both the photo ID and address ID. |
| 9. Applicant Address ID | Repeat the applicant's name and home address, then check the document used to verify that address. | The selected document must contain the address entered in 9b-9f, which should match 4f-4j. Attach a copy. |
| 10. Authorized Individual Photo ID | Complete only if Section 5 lists an Authorized Individual. Enter that person's photo ID details and ID type. | Attach a copy of the acceptable photo ID. |
| 11. Authorized Individual Address ID | Complete only if Section 5 applies. Enter the Authorized Individual's name, home address, and address ID type. | The address must match the supporting document and the address in Section 5. |
| 12. Exceptions for Additional Recipients of Mail | For Business/Organization Use, list members who will also receive mail at the PMB. | Each listed person may be required to present two valid IDs to the Postal Service upon request. |
| 13. Signature of Applicant | The individual applicant signs in 13a and enters the date in 13b. | For an LLC, an officer must sign and provide the officer's title. Use MM/DD/YYYY and follow the notarization instructions. |
| 14. Signature of CMRA or Authorized Employee | The CMRA agent or an authorized employee signs and dates this section. | Do not complete the CMRA's signature line yourself unless you are the authorized agent. |
What Happens on Page 2?
Page 2 contains the official instructions, definitions, agreement terms, Privacy Act Statement, and the notarial certificate. The applicant may sign or acknowledge the signature in the physical or virtual presence of the CMRA's authorized employee, or acknowledge the signature before a commissioned notary public as permitted by the form instructions.
If the Notary Public box is used, the notary completes the venue, date, applicant name, notary signature, commission-expiration information, and official seal. The applicant should not fill in the notary's fields.
Common LLC Mistakes to Avoid
- Putting the LLC name in every personal applicant field
- Using only one ID when two forms of identification are required
- Using an address document that does not match Sections 4 and 9
- Entering inconsistent business names or addresses
- Leaving the ID issuer or expiration date incomplete
- Using one driver's license as both required IDs
- Completing Sections 10 and 11 when no Authorized Individual is listed
- Signing Section 14, which belongs to the CMRA or its authorized employee
How Notarization Works
The notary identifies the individual applicant and completes the notarial certificate on page 2. The notary does not verify that the LLC is in good standing, approve the mailbox account, or decide whether the CMRA will accept the application.
Remote online notarization may be convenient if your mailbox provider accepts an electronically notarized Form 1583. Confirm the provider's acceptance rules before ordering. USPS and the receiving CMRA control the form and acceptance requirements.
