USPS Form 1583 ID Requirements: What IDs Are Accepted?
USPS Form 1583 requires two different forms of identification for the applicant and for any authorized individual listed on the form. One must be a government-issued photo ID. The other must verify the street home address entered on the form.
The current official form is PS Form 1583, June 2024. Its checkboxes provide the controlling list of ID categories, so use the names printed on your copy of the form and confirm any provider-specific requirements with your Commercial Mail Receiving Agency (CMRA).
Accepted Photo IDs for USPS Form 1583
Item 8e on the June 2024 form lists these photo ID types:
- U.S. state, territory, or tribal driver's or nondriver's ID card
- Uniformed Service ID
- U.S. Access Card
- U.S. university ID card
- Passport
- Matricula Consular
- NEXUS card
- Certificate of Naturalization
- U.S. Permanent Resident Card
USPS instructions state that one of your two IDs must be government-issued and contain a photograph. Because a university ID is not government-issued, do not rely on it as your only government-issued photo identification. Your CMRA or identity-verification platform may also apply narrower rules than the form itself.

Item 9g lists these address-verification documents:

The address document must contain the same street home address entered in items 9b-9f. The applicant's address in item 4f must also match the document provided for item 9b.

Can You Use One Driver's License Twice?
No. The USPS form expressly says that a driver's or nondriver's ID may be used as either the photo ID or the address ID, but not both. If you use your driver's license in item 8e, choose a separate qualifying document-such as a current lease, insurance policy, vehicle registration, voter card, or mortgage/deed of trust-for item 9g.
What About Passports and Foreign Applicants?
"Passport" is an available photo-ID checkbox on the form. The form asks for the ID number, issuing entity, and expiration date. A passport normally does not prove your current home address, so you still need a separate accepted address ID.
Applicants outside the United States should confirm with their CMRA how it handles foreign address documents, translations, and country-specific IDs before the notarization. Provider acceptance can be narrower than the USPS form categories.
Common ID Mistakes That Delay Approval
- Submitting only one ID instead of two
- Using the same driver's license for both categories
- Providing an address document that does not match items 4f and 9b-9f
- Using an expired or unreadable ID
- Cropping, blurring, or covering important ID information
- Choosing a bank statement or utility bill without first confirming acceptance
- Entering the wrong issuing entity or expiration date in items 8c and 8d
